The representative must be one the following:

(a) A Plan Nominee as defined by the NDIA

(b) A Correspondence Nominee as defined by the NDIA

(c) A Child Representative as defined by the NDIA

(d) A person authorised to act for the participant under a current, valid and operative power of attorney

(e) A person appointed as the guardian or manager of the participant by Court/Tribunal Order
